Step 1. Gather supplies or call our Customer Service Team at 877.283.5351 if you don’t have the supplies to send your case.

  • Our customer service representative will send everything you need to get started and will continue to send supplies when needed.
  • The kit will include: boxes, pre-printed* UPS labels and RX forms.
    *If you need next day air labels for rush cases, they are available upon request.
  • Re-order supplies can be sent immediately or with your next case per your request.

Download and complete the RX form & prepare for shipping.

  • Click here if you’re sending a digital impression
  • Multiple cases may be sent in one box
  • Place each impression into a plastic bag and staple the RX form to the outside of the bag for accurate identification
  • Tape box securely
  • Attach UPS label to the top of the box and call us. We will schedule your pick-up
    *We will provide UPS bags upon request
